What should I include in my vacation rental pet policy?

I want to allow pets at my rental, but I want to make sure I've covered all my bases so we don't have any incidents.

The pet policy should define what type of animals will be allowed. Not everyone will show up with a cat or a dog. If you don't want a pet pig in the home or have to deal with an escaped rodent or snake, specify what type and size of pet is allowed and the responsibilities of the pet owner.
The breed of the animal can also be specified, along with special requirements you feel are needed as to when the pet is left alone. These can be based on the pet provisions that you provide and the size of the rental. For instance, do you have a fenced in area or must the pet be leashed when in the yard? 
Additional rent and a damage and security deposit should also be addressed in the policy.
